The main risk associated with this procedure is that it may cause an asthma attack. This possibility is reduced by treating you with albuterol before beginning the hypertonic saline, and by monitoring your lung function during the test. The test will be stopped after you produce an acceptable sputum sample, or sooner, if your lung function worsens to an unacceptable level or if you choose to stop the test. Risks with study drugs. Changing your asthma medications. When you participate in this study your asthma treatment might change. You will be put on a standard dose of an inhaled corticosteroid, and then have tiotropium added, salmeterol added, and have the inhaled corticosteroid dose increased at different times. 171 THE FOLIC ACID COMMUNITY PHARMACIST EDUCATION TRIAL FACT ; : A PILOT STUDY S Bjelajac PharmD, L Dolovich PharmD MSc, Z Austin PhD, McMaster University and the University of Toronto BACKGROUND: Lack of knowledge about or use of periconceptual folic acid has been associated with an increased incidence of neural tube defects. Despite many recent educational efforts a substantial proportion of women in Canada are still unaware of the recommendation for folic acid in pregnancy or did not take them prior to conception. OBJECTIVE: To determine the effects of a folic acid training workshop pilot for pharmacists on Pharmacist Self-Reported Knowledge And Skills. METHODS: This was a pre-post pilot study. Included pharmacists were volunteers who responded to advertisements in pharmacy journals or to mailed invitations. Participants attended a five-hour workshop that contained the following elements: needs assessment and immediate post workshop evaluation, didactic presentations, patient simulations, interactive case discussions, and feedback debriefing. A questionnaire was administered to ascertain the self-reported knowledge 10 point scale pre and post ; and skills post only ; . A paired t-test was used to test differences in knowledge before and after the workshop. RESULTS: Seventeen pharmacists participated in the workshop. Knowledge scores changed from 4.7 SD, 2.3 ; before the workshop to 7.5 SD, 1.4 ; after the workshop p 0.0001 ; . Sixteen learning needs were identified as addressed by the workshop. Twelve participants 71% ; reported that they felt more confident in applying the information to practice. Simulated patient role play was most commonly identified as the most useful workshop component. CONCLUSIONS: The workshop improved pharmacists' short term self-reported knowledge and met numerous learning needs. And then, there's a big blue area here. Marketing Advertising Administration. This is the fastest growing area of expenditure in drug companies. And how many of you have seen those drug ads on TV? Have you seen some of those? They're spending about 30 cents out of the dollar on drug ads. Now, how many of you got 30 percent better after you saw that? Are we getting our moneys worth from those drug ads? Are you getting better? Is it worth the 30 cents? Every time you spend 0 on prescriptions, you've spent on advertising for those TV ads. Did you enjoy it? Did you like the entertainment? Now, we often hear that if pressure is put on the drug industry, that their profits will go away and we won't have any new drugs, and we can't cure your grandmother or your grandchild. Well, what's happened over time to net profits? This is Fortune 500 net profits from 1970 to 1999. And then I saw that drug firms are the red line, the median Fortune 500 is the blue line, and pharmacies are the green line. Now, who's responsible for the greatest increase here again? Who has the greatest profits? Is it the pharmacies? No, it's not corporate America. Drug companies are the most profitable industry according to Fortune 500, and have been for more than 20 years, and the gap keeps getting wider, not narrower. And notice the Hatch-Waxman Act passed. Did drug company profits go down after that? No. Over 90 Medicaid rebates were passed. Did drug company profits go down? Well, it looks like it did in the next year but, actually, notice that all industries went down that next year. So that dip is across all industries, not just a drug phenomenon so I don't think we can say it's due to Medicaid rebate. And then, after that, what's happened to drug firm profits? They continued going up. Is price still an issue? Well, I've looked at recent drug price changes. This is a set of prices from January of 2001 to January of 2002, just a couple of weeks ago. What do we see with--and during this time period, the consumer price index in the last year increased about 2.8 percent. What happened to Daypro? Eight percent increase. Now, how many of you had an 8 percent increase in salary last year? You didn't? Well then, this is taking a bigger bite out of your pocket and, just wait, it gets more exciting. Enbrel is another medicine for arthritis, 9.3 percent increase in the last year. Celebrex, 10.4 percent. Now, you've seen a lot of that on TV and, yes, you're paying for it. What about the respiratory inhalers? Atrovent went up 10.2 percent. Combivent went up 10.2 percent. Serevent went up 10.2 percent. Now wait a minute, there's a pattern here. Did all these companies compare prices and raise them the same? I think somebody needs to be looking at.

Improvement in FEV1 during the course of either study, thus showing that patients do not become tolerant of the effects of 5-LO inhibition. Studies using compounds that block the effects of leukotrienes both LTRAs and 5-LO inhibitors ; have shown that asthma control is improved in adults and in children older than 12 years of age. There is some evidence that antileukotrienes may be even more effective in patients with severe asthma. Their effect added to the bronchodilation achieved even with high doses of inhaled 2-agonists31, 32 suggests that they may have a place in the treatment of the severe bronchoconstriction associated with acute severe asthma, although this has not been evaluated in clinical studies. A clinical benefit of their addition has also been demonstrated in patients with poor asthma control, who are already taking high doses of inhaled glucocorticosteroids.33 In one study, 34 the receptor antagonist pranlukast prevented asthma exacerbations in patients in whom the doses of inhaled glucocorticosteroids were reduced by half. Two studies22, 23 that specifically assessed the role of antileukotrienes in patients with aspirin-sensitive asthma showed that these drugs effectively blocked the ASA-induced asthmatic responses. WARNINGS AND PRECAUTIONS Risk of Asthma-Related Death With Long-Acting Beta2-Adrenergic Agonists Long-acting beta2-adrenergic agonists, such as salmeterol, one of the active ingredients in ADVAIR DISKUS, may increase the risk of asthma-related death. Therefore, when treating patients with asthma, physicians should only prescribe ADVAIR DISKUS for patients not adequately controlled on other asthma-controller medications e.g., low- to medium-dose inhaled corticosteroids ; or whose disease severity clearly warrants initiation of treatment with 2 maintenance therapies. A large placebo-controlled US study that compared the safety of salmeterol with placebo, each added to usual asthma therapy, showed an increase in asthma-related deaths in patients receiving salmeterol. The Salmeterol Multi-center Asthma Research Trial SMART ; was a randomized, double-blind study that enrolled long-acting beta2-agonistnaive patients with asthma to assess the safety of salmeterol SEREVENT Inhalation Aerosol ; 42 mcg twice daily over 28 weeks compared with placebo when added to usual asthma therapy. A planned interim analysis was conducted when approximately half of the intended number of patients had been enrolled N 26, 355 ; , which led to premature termination of the study. The results of the interim analysis showed that patients receiving salmeterol were at increased risk for fatal asthma events see Table 1 and Figure 1 ; . In the total population, a higher rate of asthma-related death occurred in patients treated with salmeterol than those treated with placebo 0.10% vs. 0.02%, relative risk 4.37 [95% CI: 1.25, 15.34] ; . Post-hoc subpopulation analyses were performed. In Caucasians, asthma-related death occurred at a higher rate in patients treated with salmeterol than in patients treated with placebo 0.07% vs. 0.01%, relative risk 5.82 [95% CI: 0.70, 48.37] ; . In African Americans also, asthma-related death occurred at a higher rate in patients treated with salmeterol than those 5.
Geriatric Use Of the total number of adolescent and adult patients with asthma who received SEREVENT DISKUS in chronic dosing clinical trials, 209 were 65 years of age and older. Of the total number of patients with COPD who received SEREVENT DISKUS in chronic dosing clinical trials, 167 were 65 years of age or older and 45 were 75 years of age or older. No apparent differences in the safety of SEREVENT DISKUS were observed when geriatric patients were compared with younger patients in clinical trials. As with other beta2-agonists, however, special caution should be observed when using SEREVENT DISKUS in geriatric patients who have concomitant cardiovascular disease that could be adversely affected by this class of drug. Data from the trials in patients with COPD suggested a greater effect on FEV1 of SEREVENT DISKUS in the 65 years age-group, as compared with the 65 years age-group. However, based on available data, no adjustment of dosage of SEREVENT DISKUS in geriatric patients is warranted. ADVERSE REACTIONS Data from a large, 28-week, placebo-controlled US study that compared the safety of salmeterol SEREVENT Inhalation Aerosol ; or placebo added to usual asthma therapy showed an increase in asthma-related deaths in patients receiving salmeterol see WARNINGS and CLINICAL TRIALS: Asthma: Salmeterol Multi-center Asthma Research Trial ; . Asthma Two multicenter, 12-week, controlled studies have evaluated twice-daily doses of SEREVENT DISKUS in patients 12 years of age and older with asthma. Table 4 reports the incidence of adverse events in these 2 studies.
